Additionally, Jericho Road was selected as a New York State Delivery System Reform Incentive Payment (DSRIP) Program partner, due to its health service innovations that contribute to State goals of reforming Medicaid and reducing avoidable hospital visits by 25%. Jericho Road is a NYS Office of New Americans Opportunity Center, one of 27 centers in the State that provides ESOL, naturalization, and employment support for refugees. Jericho Road has achieved the highest certification (Level 3) as a Patient-Centered Medical Home from the National Committee for Quality Assurance. HRSA has recognized Jericho Road’s Hope Refugee Drop-In Center, Priscilla Project, and Care Coordination services as model best practices for community health centers. Jericho Road Community Health Center is a New York State Article 28 Diagnostic and Treatment Center accredited by the NYS Department of Health, and a Federally Qualified Health Center regulated by the Health Resources and Services Administration (HRSA), an agency of the U.S. The two organizations merged in 2013 to serve the community more holistically and more sustainably as a Federally Qualified Health Center. JRM was founded in 2002 to provide more integrated care through programs that address non-medical, poverty-related barriers. Myron and Joyce Glick in 1997 as a small family practice, JRFP has provided safety net healthcare for low-income and refugee community members, targeting areas of concentrated poverty in Buffalo. Jericho Road has a shared history as Jericho Road Family Practice (JRFP) and Jericho Road Ministries (JRM). Jericho Road Community Health Center (JRCHC) provides primary healthcare and supportive programming to low-income residents and refugees in the City of Buffalo. Our vision is for underserved and marginalized individuals, families, and communities in Buffalo and across the world to become healthy and whole. Jericho Road Community Health Center provides wholistic healthcare for the underserved and marginalized communities in Buffalo and across the world in order to demonstrate the unconditional love of Jesus.
